The MFU hotel feedback form is to be used by Mixed Fleet Unite members to report hotel incidents and provide crew hotel feedback.
By completing this form it allows the Mixed Fleet Unite Hotels Officer and the Health and Safety Reps to continually monitor and collect hotel feedback and incidents so we can work with British Airways to find a resolution.
In order for your complaint to be actioned efficiently, as well as completing this form, you must:
1 - If an incident does occur within a hotel you should report it to British Airways at the earliest opportunity. You can do this by completing an incident form via my.baplc.com by clicking on the IFCE Incident Form on the left-hand sidebar. Alternatively, you can write out an IFCE incident form and then post it in the 'IFCE Incident Form' box in the IFR banking room or in the Crew Report Centre. This must be completed within 72 hours of the incident happening.
2 - Complete the Crew Hotel and Transport feedback form found on the BA intranet under the 'crew hotels' section or by clicking this link:-
3 - If you are down route away from base and unrested due to an incident then you must inform the SCCM/Captain and speak to the IFCE Duty Operation Managers (DOMS).
There are 2 parts to this form depending on the situation.
You will be asked what is the nature of your feedback, based on this you will be directed to the relevant part of the form to complete.
Part 1- Is for general feedback about the hotel such as location, amenities, staff, room location and HOST.
Part 2 - Is for incidents which have occurred whilst in the hotel such as, noise disturbance, theft from rooms and any other safety-related incidents.

REMEMBER: You need to be an MFU member to submit a form and have your complaint actioned.
